Loyalty Program Terms & Conditions
1. Eligibility
The Dr. Z. Paul Lorenc Aesthetic Clinic Loyalty Program (“Program”) is open to individual patients aged 18 and older. By enrolling, participants agree to these Terms & Conditions and to the clinic’s Privacy Policy.
2. Program Overview
The Loyalty Program is designed to reward loyal patients who spend $3,000 or more within a rolling 12-month period at Dr. Z. Paul Lorenc’s practice.
Eligible participants will receive exclusive rewards, offers, and perks as determined by the clinic.
3. Rewards and Benefits

Rewards are earned based on total eligible spend within the clinic.

Reward tiers, benefits, and offers are subject to change at any time without notice.

Rewards hold no cash value and cannot be redeemed for cash or transferred to another patient.

Rewards cannot be combined with other promotions, discounts, or offers.

Loyalty perks and seasonal offers may vary depending on availability and clinic discretion.

Not applicable to clinical trial patients.
4. Program Duration and Inactivity

The Loyalty Program membership remains active as long as the participant engages in qualifying activity (purchases or services) at least once every 12 months.

If there is no qualifying activity for one (1) full year, the membership will automatically expire, and any unused rewards will be forfeited.
5. Program Modification
Dr. Z. Paul Lorenc Aesthetic Clinic reserves the right to modify, suspend, or terminate the Program at any time without prior notice. This includes changes to reward thresholds, benefits, or eligibility criteria.
6. Non-Transferability
Membership and rewards are personal to the patient and may not be sold, gifted, or transferred to another individual.
7. Fraud & Misuse
The clinic reserves the right to revoke membership or rewards in cases of suspected fraud, abuse, or violation of these Terms & Conditions.
8. Privacy
Participation in the Program constitutes consent to the collection and use of personal data as described in the clinic’s Privacy Policy. Data is used solely for Program administration and communication purposes.
6. Limitation of Liability
Dr. Z. Paul Lorenc Aesthetic Clinic is not responsible for any loss, damage, or injury arising from participation in the Program, including errors or technical malfunctions.
7. Governing law
These Terms & Conditions are governed by and construed in accordance with the laws of the State of New York.
